Clinical Quality Improvement

WACMHC is committed to improving the quality of healthcare for all Washingtonians by assisting community health centers target health disparities using quality improvement methodologies. We offer resources and continuous support and training for quality improvement initiatives, lend expertise in areas related to process improvement and create opportunities for leadership and networking among health center quality professionals.

Our team currently provides direct support to 6 FQHCs to improve colorectal cancer screening rates through a grant opportunity in partnership with the Centers for Disease Control (CDC), Washington State Department of Health, and the Alliance for Reducing Cancer NW at the University of Washington.

Quarterly Quality Improvement Roundtable Training

WACMHC regularly convenes quality improvement and clinical staff for in-person and virtual training events tailored to the current needs expressed by our members. QI Roundtables facilitate learning through subject matter expert consultation and peer-to-peer activities to cultivate best practices.Past QI Roundtables have included:

  • Lean Six Sigma: White Belt Training
  • Medicaid HER Incentives and Clinical Data Sharing: The Path Forward in 2017
  • Strategies to Improve Hypertension and Diabetes Care in Washington State

HRSA Site Visit Guidance & Support

WACMHC helps members comply with HRSA's Health Center Program Requirements and prepare for their annual HRSA site visit via ongoing in-person support from a pre-visit self-assessment through closing remarks. WACMHC provides centers with a 19 requirements self-assessment and health center common findings and recommended solutions.

Patient-Centered Medical Home (PCMH)

The patient-centered medical home (PCMH) is a framework for delivering primary care that enables practices to provide comprehensive, patient-centered, coordinated, accessible, high-quality and safe healthcare. The PCMH model has the potential to improve clinical quality, patient experience and reduce health system costs.
