Practice Transformation

The Washington Association of Community & Migrant Health Centers (WACMHC) is committed to supporting the efforts of Washington health centers in promoting health and access to high quality healthcare for all Washingtonians. Our Practice Transformation staff offer training and technical assistance to support health centers in the delivery of evidence-based care that is culturally appropriate for their patients. We assist and support health center staff through multiple channels including one-to-one coaching, peer-to-peer learning, training events, and meeting facilitation.

The Practice Transformation Team participates in national and State efforts to reduce health disparities, emphasizing colorectal cancer screening and diabetes and hypertension management, by engaging with partners such as the DOH and the Diabetes Network Leadership Team.

Our team currently provides direct support to 6 FQHCs to improve colorectal cancer screening rates through a grant opportunity in partnership with the CDC, Washington State DOH and the University of Washington.
